Cold Laser Therapy for Carpal Tunnel Syndrome: Manhattan, New York, NY;

With the recent arrival of our new irradia cold laser – a 904 superpulsed infra red laser – the influx of inquries has been massive. People in manhattan, new york, have been calling the office to learn if cold laser therapy is effective for carpal tunnel.

The simple answer: YES!

However, the one thing I always tell patients is that any therapy is only as good as their diagnosis. Let me elaborate. When a therapy is proven effective for treating a certain condition you want to be sure that if you ‘think’ you have this condition that you have it properly diagnosed.

Providing an effective treatment for a misdaignosed condition will never work. You need to know what you’re dealing with in order to make good clinical decisions for care. If a patient, which most do, self diagnoses themselve off of information they found on the internet they may be excited about a potential therapy, and rightfully so.

Often times I see patients with symptoms that present similar to that of carpal tunnel but in fact is not carpal tunnel syndrome at all. There are a few things that can mimic such a condition. More often then not we see a few complicated conditions that may in fact have a ‘component’ that is actually carpal tunnel but the original injury may have been sustained in the neck region.

This will present as a double crush syndrome wehre there is not only compression of nerver fibers in the wrist but also the neck. This type of presentation won’t respond to simple therapy at the wrist alone. You need to address the secondary (in this case actually the primary) comression first – at the neck.

In addition to a double crush syndrome some patients thinking that they are suffering with carpal tunnel actually have cervical radiculopathy – or symptoms in the arm, wrist and hand – that are actually the result of compression at the neck. Sometimes this is the result of a herniated disc or arthritis in the cervical spine.

So, once again, is cold laser therapy effective for carpal tunnel? Yes, absolutely. Just make sure that you’ve been properly evaluated and diagnosed with the right condition – this will utlimately determine the likelihood of a full and proper recovery.

The 2-minute Sciatic Nerve Press: Significant Pain Relief Using Sciatic Nerve Pressure Points,Sciatica NYC

The following is a reprint of an article covering recent research that showed a 2-minute ’sciatic nerve press’ provided significant pain relief, not just for those suffering with sciatica, but those suffering with pain raning from cancer pain to dental pain.  The researcheers concluded that this procedure was effective but are not quite sure why, yet.

KISSIMMEE, Fla., Feb. 18 — Two minutes of pressure on the path of the sciatic nerve appears to bring up to an hour’s relief of pain and discomfort for people with a variety of symptoms.

Pain relief averaged 43.5% (P<0.001) for emergency department patients, Jiman He, Ph.D., a visiting scholar at Brown University in Providence, R.I., told attendees at the American Academy of Pain Medicine meeting here.

Significant pain relief (P<0.001) was also achieved during an hour-long observation for a combined group of dental, renal, and cancer patients, Dr. He said.

“We really don’t know why this works,” said Dr. He, who used aspects of traditional Chinese medicine to determine pressure sites that give relief even if a clinician does not have expertise in acupuncture.

In a separate presentation, Dr. He noted that pressure on the sciatic pathway also brought relief of nonpainful discomfort, including fullness, bloating, nausea, and vomiting, among cancer patients (51.9%, P<0.001) and patients at an internal medicine clinic (37.3%, P<0.05).

Dr. He determined that pressure on the sciatic nerve in the back of the thigh appears to be part of a feedback mechanism that relieves discomfort in diverse areas of the body.

“Overall, about 43.5% of pain in 65 patients was relieved by the sciatic press,” Dr. He, whose permanent residence is in Beijing, said. “About 80% of patients with dental pain get relief from the sciatic press and about 60% of patients with kidney pain achieve some relief.”

Another 70 patients underwent a placebo press point on the front of the legs. Their pain scores on a visual analog scale decreased from 7 to about 4.5 over the course of one hour. The patients who received the sciatic press saw their pain scores drop from 7 to about 3 after one hour. That difference reached statistical significance (P<0.001), Dr. He said.

Dr. He, who is now recruiting 2,000 patients for a definitive test of the press, explained that the press is performed with the patient lying on his or her stomach. With the palms or the fist, a clinician presses on the general area of the sciatic nerve from the buttocks to mid thigh.

 By exerting 11 kg to 20 kg pressure on the area for two minutes, the patient’s discomfort is relieved, Dr. He said.

“We found that there are about 30% of people for whom the technique fails to work at all,” he said. The long-term study will attempt to figure out for whom the press works and for whom other techniques are required.

“We are also going to attempt to determine the mechanism of action for the pain relief that we see,” Dr. He said.

“This is a fascinating study,” said Joel Konikow, M.D., a private practice pain medicine specialist in Seattle. “I might try it in some of my patients with acute pain. I am concerned about its long-term utility with chronic pain patients because, as Dr. He said, repeated use of the technique results in diminished length of time of relief.” 

Dr. Konikow, who discussed the treatment with Dr. He at the poster presentation said, “This paper is intellectually challenging in that it has a lot to teach us about how to heal pain. It shows us that pain is nonlinear. Just because it hurts in one place doesn’t mean that treating that area will heal the pain.”

 Dr. He said that applying pressure anywhere along the sciatic nerve will relieve the pain, but pressing on the sciatic nerve in the calf, for example, has less of an effect than pressure in the area higher on the leg.

Pressing on the leg for less than two minutes will also bring relief, Dr. He said, but it is less long acting. He said that pressing on the leg for more than three minutes does not add to pain control.

 He also said that use of fingers or the thumb to press on the nerve may cause the practitioner to miss the nerve, and not enough pressure can be exerted with the fingers.

Low Back Pain New York City, The 5 Minute Core Muscle Treatment: ATM2

The core muscles of the spine are an often overlooked component of low back pain in the clinical setting. However, these simple group of muscles can be the difference between a painful range of motion and one that is pain free, literally, in a matter of minutes.

If you imagine your trunk as an onion with layers of rings from the core all the way to the outside this will represent our muscle support network. Typically, the center of the onion is engaged and supports the spine. This provides our brain with a feeling of security, i.e., stability, which allows for us to excute a full range of motion across that particular joint. In simplest terms we require stability for mobility.

Mobility without stability is what leads to repetitious stress, degeration and injury.

If the center of your onion is weak your brain will naturally look for help in providing the much needed stability. So, what does it do? You guessed it. The brain will start to recruit other layers of the ‘onion’ and squeeze the inside, ‘core’, from the outside in.

Can you begin to see how inefficient this is? What’s more is that these muscle groups were not designed to fire all day and support the spine. This is why they begin to fatigue, ache and cause you eventual pain.

It is these core muscles of the spine that the ATM2 was designed to rehabilitate and condition.

The ATM2 is a clinical device designed specifically to enable clinicians to prescribe quality user operated ATMs, i.e. weight-bearing active movements towards the previously impaired movements. Passive joint repositioning and stabilizing is obtained via restraining belts connected to the ATM2 support pad to reduce the symptoms, and then specific ATMs are performed via a harness connected to a resistance band.

ATMs are a new approach to treating musculoskeletal disorders. Their specificity and immediate effect provide new avenues in the professional outpatient and sports medicine setting. What’s unique about the ATM2 concept is that once the user is transitioned to a pain free movement and full range of motion is restored continued training on the device leads to plasticity within the CNS that ‘imprints’ the new movement pattern into memory.

This process is automatic and requires no volitional movement from the end user. Once these muscles are engaged, sequenced and reactivated the muscles may be continuously conditioned through routine training sessions on the ATM2 leading to an enhanced level of performance.

In conclusion The ATM2 can improve muscle activation patterns in just a few minutes, which leads to the following immediate outcomes:

1. Immediate increase in ROM: Increases efficiency and usage of the core stabilization muscles (multifidus/transverse abdominus) causes immediate relaxation of global movement muscles which result in immediate increase in ROM, with out ever doing inefficient and potentially harmful stretching exercises.

2. Immediate increase in strength: The ATM2 causes muscle sequencing to become more efficient in that less muscle activity achieves stronger more sustained outputs.

3. Immediate reduction in pain: Inefficient and wasteful noise related to the human body being inherently unstable (in the upright, functional position) is significantly diminished – thus more efficiency of movement which leads to more strength and better movement across joints. Furthermore, the ATM2 causes muscle activity to balance out with in the body and facilitate painful movement to become totally pain-free by transition the CNS from a painful movement strategy to one that is free of pain.
